It’s hard to believe that a fax cover sheet template is still needed!
Just recently, I was required to fax information. Granted, it was to a governmental organization. That probably explains why it was a fax and not an email.
I hadn’t sent a fax in years.
People seem to go to the fax machine when 1) privacy is important, 2) physical documents are required, or 3) the industry/organization hasn’t caught up with technology.

How To Use The Fax Cover Sheet
- Download the fax cover sheet. You can download this free excel template by clicking the download link above.
- Edit the template. There are two tabs. The first tab is a blank template. The second tab is an example. Just complete the information required, and you’ll be set. You can print this off and hand write everything, or you can enter it in excel. You will want to put your logo in and type your company/personal contact information.
- Put the cover sheet on top of all of the documents you need to fax and you’re off!

Do you need to send a fax but don’t want to go through the trouble of doing it all yourself? Look for a UPS, FedEx, or a local office supply store near you. Call before you go to ensure they’ll be able to help you and to figure out the cost. Oftentimes, the setup they have will be exactly what you need to send the occasional fax.
