Description: The National Heart Lung and Blood Institute states that 1 in 3 American adults has high blood pressure.    Use this log to keep track of your blood pressure readings and monitor your progress making improvements in your blood pressure levels.  


This is a monthly log, allowing for up to 35 readings.  The graph at the top of the log will automatically update as you enter your readings.  We've included ranges of "Desirable" blood pressure readings.  Please consult with your personal physician for blood pressure levels acceptable for your individual circumstances.
